What does it mean to provide person-centered care?  Participants learn what being "person-centered", the benefits of person-centered care for residents, and how to implement person-centered care. 

 

Memory Care Track: Transitioning Successfully: What You Need to Know!

How well do you transition your residents between homes or levels of care? Do you wait the right amount of time between transitions? How do you know what the right amount of time is, or how to effectively implement and complete these transitions? Attend this track and learn the best ways to successfully transition your residents into your facility, out of your facility, or between levels of care within your residence.

 

Mental Illness Care Track: Psychiatric Emergency Care: Crisis Management

Colorado Crisis Services are for ALL Coloradans experiencing a mental health or substance abuse crisis. In this session you will learn several ways you can access mental health crisis help through private and state services. You will also learn techniques you and your staff can use prior to your client's mental health crisis intervention.

The Federal Center for Medicare and Medicaid Services (CMS) has issued new home and community based service setting regulations. The HCB Settings Final Rule has implications for both residential and non-residential service settings which will be implemented over the next 4 years. Come learn what CDHCPF and the stakeholder group have accomplished and get prepared to be compliant.

Assuring that people are trained and knowable in QMAP is required for every assisted living facility. Learn about changes in the program and how you are going to adjust to the programming.

Understanding the safe handling and storage is paramount in the management of an assisting living facility. To avoid getting a bad survey come listen to this important program.

Memory Care Track: Managing Difficult Resident Behaviors: Positive Intervention Strategies.

Participants will learn behavioral intervention strategies to help them positively address challenging behaviors. Prohibited procedures are also reviewed.

Introduction to QMP: All Your Questions Answered

By the end of 2015 all Colorado assisted living facilities will be required to have a Quality Management Program in place for their residence. What does this mean? What does it look like? What kind of resources will it take to implement this seemingly overwhelming task? You will find out in this program.

QMP Program – Part 2

3:00pm to 3:50pm - Summit Ballroom, 5th Floor

 QMP Deep Dive Breakouts

During our afternoon breakouts, we will sort through what we have learned about Quality Management Programs: how to create and implement them, what a QMP looks like for a small home versus a large residence, and how to effectively follow through with your program.
